Change of Program

 

A student who wishes to change his or her program/major must complete the top portion of the Change of Program form. This form may also be used to declare a minor(s), a double major, or to change the catalog year.
 
The transferring department completes the middle section of the form, releases the counseling file to the student to take to the new department, and keeps the pink bottom copy of the form. The receiving department completes the bottom portion and sends the white copy to the Registrar's Office to be processed. The yellow copy is kept in the student's counseling file by the receiving department.
 
The receiving department is responsible for expiring any advising holds placed by the transferring department and adding an advising hold, if applicable. Also, the old advisors must be expired and the new advisors assigned. Any student attributes that need to be deleted or added must be indicated on the form for the Registrar's Office to update.
 
Changes are effective the current term. The only exceptions would be for students accepted into the professional component of a program, such as in Allied Health or Nursing.
 
Note: Students changing or declaring a major or minor in music must be able to read music and must complete an audition for admission to the program.
