Open Enrollment for the USA Health & Dental Plan, USA VIVA Health & Dental Plan, and SouthFlex

Open Enrollment for the USA Health & Dental Plan, USA VIVA Health & Dental Plan, and SouthFlex begins on Wednesday November 1, 2017. If you do not wish to make any changes to your health/dental insurance, no action is required. Employees should have received the HR Advantage at their University issued e-mail address and at their home address, which provides detailed information about benefit updates, and other items of interest. For new enrollment effective January 1, 2018, premiums will be deducted from bi-weekly paid employees in the month of December since premiums are paid one month in advance.

The deadline for submitting changes and enrollments is 4:30 p.m. on Thursday, November 30, 2017. After this date, forms for open enrollment will not be accepted and the web links for open enrollment will become inactive. All changes and elections will become effective on January 1, 2018. Contact Human Resources at (251) 460-6133 if you have any questions.

The USA Health & Dental Plan enrollment/change form (administered by Blue Cross Blue Shield of Alabama) is available online and in the Human Resources offices. You may scan and e-mail the completed enrollment form to hrmaincampus@southalabama.edu. Forms will not be accepted by facsimile. If you send your form by e-mail, it is not necessary to return the original form. 

The USA Health & Dental Plan prescription drug benefit will continue to be managed by Express Scripts, Inc (ESI).  ESI is the number one ranked pharmacy benefit company nationwide.  They are able to provide more cost effective pricing and medical care management which results in an overall better benefit for all plan members.  Additional information may be found below.

The claims administration of SouthFlex will continue to be managed by HealthEquity®. We will also continue to offer the HealthEquity® Visa® Reimbursement Account Debit Card. When you incur a qualified medical expense, you will be able to pay with the HealthEquity® Visa® Reimbursement Account Card or submit the expense through the HealthEquity online tool for reimbursement. Human Resources offices.

You may scan and e-mail the completed enrollment form to hrmaincampus@southalabama.edu. Forms will no longer be accepted by facsimile. Remember, you must enroll each year to participate in SouthFlex. Even if you are currently enrolled in SouthFlex, you must submit a new enrollment form to participate in 2018.

We are pleased to announced that your USA Fringe Benefits Committee (FBC) recommended, and President Tony G. Waldrop approved, no increase in the USA Health and Dental Plan (BCBS) employee premiums:

USA Base Plan monthly premiums: (employees hired before 1/1/2013):

Single coverage : $108.00 ( $158 without the non-tobacco wellness incentive)

Family Coverage : $354.00 ( $404 without the non-tobacco wellness incentive)

USA Standard Plan monthly premiums: (employees hired on or after 1/1/2013):

Single coverage : $128.00 ( $178 without the non-tobacco wellness incentive)

Family Coverage : $418.00 ( $468 without the non-tobacco wellness incentive)

The University of South Alabama in coordination with USA Health offers employees the opportunity to participate in the, USA VIVA Health & Dental Plan. USA VIVA Health & Dental Plan offers value to employees and their dependents with a low cost benefit plan that provides access to select medical providers in a narrow network with a concentration on improving medical outcomes and providing preventive health care.
