Survey Requests

Requests to send surveys to the campus community must be approved by the Office of Institutional Effectiveness. Below are instructions for submitting your survey for approval.

Step #1 - Review the Survey Policy

 
Step #2 - Email survey requests to effectiveness@southalabama.edu
Please include:
  • a copy of the survey,
  • IRB approval;
  • administration dates;
  • supervising faculty member and/or course information (student survey requests);
  • to whom the survey should be sent (e.g., all faculty/staff, all student, both);
  • requester's contact information (email and phone number); and
  • a copy of the e-mail that will be sent to recruit participants.
