Requests to send surveys to the campus community must be approved by the Office of Institutional Effectiveness. Below are instructions for submitting your survey for approval.
Step #1 - Review the Survey Policy
Step #2 - Email survey requests to firstname.lastname@example.org
- a copy of the survey,
- IRB approval;
- administration dates;
- supervising faculty member and/or course information (student survey requests);
- to whom the survey should be sent (e.g., all faculty/staff, all student, both);
- requester's contact information (email and phone number); and
- a copy of the e-mail that will be sent to recruit participants.
