The electronic documents below (with the exception of the Resource Information Form) must be downloaded, filled out, signed, printed, and FAXED to the office at (251) 460-6517 or mailedto the Financial Aid Office. They cannot be e-mailed. The Resource Information Form is completed online and does not need to be printed. Send these documents only if you have filed the FAFSA for the applicable year and USA has received the results of that filing.

To see if we have FAFSA data on you, check PAWS or contact our office.

Any documents received prior to our receiving your FAFSA data will be destroyed without notification.
