Online Parent Journal
For Children with Special Health Care Needs
 
 
The Parent Journal was developed after identifying barriers to care among children with special health care needs. One barrier identified was a lack of communication and understanding between parents of children with special health care needs and their health care providers, and among the health care providers themselves. This journal will enable you to keep up-to-date, accurate information on your child's ongoing medical conditions as well as activities of daily living that will be of interest to anyone involved in the care of your child. The pages of this journal cover everything from emergency information to comfort needs. Please print any sections that you feel will be of use.
