How do I become a CHA?

Anyone can be an advocate for health in their community. To be part of the Mobile CHA team, just download, print, and fill out the short profile found here:

Once you have filled it out, bring it to the Center of Excellence office at 578 Stanton Rd (across Stanton from the USA Medical Center) or mail it to:
