Registration for the symposium is $50 per person. Fees may be paid by check, money order, or purchase order. Completed registration forms must be received no later than July 10th, 2007 along with full payment.
To register for the symposium follow the steps below:
1. Download the RHDS brochure: RHDS_Brochure.pdf
2. Print the brochure and fill out the registration form inside.
3. Mail the completed form along with your payment to:

Late Registration and Refund Policy
Late or on-site registrations will incur a $25 administrative fee. Cancellations received by fax, mail, or email on or before July 10th will receive a full refund minus a $10 administrative fee per attendee. Cancellations received after July 10th, including "no-shows" are ineligible for a refund
