Usually, all providers of health care will bill HTH Worldwide directly for services provided to Insureds. This is the preferred procedure – you are not bothered with claim forms, and HTH Worldwide needs more details than are ordinarily provided on bills to patients.
But sometimes a physician may not bill HTH Worldwide, or an ambulance company may send the bill directly to you. Also, pharmacies will not bill HTH Worldwide. In these instances, HTH Worldwide has no way of knowing about your claim. So, you must mail the bills to HTH Worldwide within 90 days of treatment and include a claim form. Claim forms are available at www.hthstudents.com. You are urged to send HTH Worldwide each bill immediately upon receipt.

If there is a difference between this program description and the certificate wording, the certificate controls.
