Exit Survey Instructions

1. Log onto the Internet using Internet Explorer

2. Go to http://usaonline.southalabama.edu

3. Click on the Login button: on the left of the screen.

4. Enter your complete USA Jag student ID number.

Example: J00212345

5. Enter your password, which will be the last four digits of your social security number.

This will take to your Personal Home Page.

Scroll down if necessary and click on the link to your Exit Survey.

Click on the appropriate eComapnion for your internship, or for

SED 464 & IDE 597, select the appropriate evaluation module.

6. Complete the survey following the on-screen instructions.

When you have finished the survey:

7. Click Save Responses

8. Then click Done With Survey.
