Objective: You will be able to design a survey, collect responses for it, and analyze its’ results.

 

ACTIVITY

 

 Look at the following survey (Figure 1). We are going to learn how to create this survey using Survey Monkey.

Working with Survey Monkey

 

1. Starting Survey Monkey

  1. Using any standard web browser (internet explorer/netscape)
  2. Type <www.surveymonke.com> in the address bar
  3. Under e-mail address type jdempsey@usouthal.edu (Figure 2)
  4. Under password type (wait for assistant to type) remember the pass word is case sensitive
  5. Click <Enter>

2. New Survey (Figure 3)

  1. Click <New Survey>
  2. Check <From Scratch>

3. Theme: (Figure 4)

  1. Click on the “(v) arrow” next to Theme for the color of the complete survey
  2. Theme Editor will open up choose your color, I chose <dewy meadow> (Fig 5)
  3. Click <Add>

4. Edit the Survey title (Figure 6)

  1. Title <JagTran Service>
  2. You can leave the other fields same or can change to your own personal interest types to distinguish previous, next, done, and for exiting the survey.

  4. Click <Update>

5. Edit Numbering (Figure 7, 8)

  1. Click <Edit numbering>
  2. Select <Auto-number pages>
  3. Select <Auto-number questions>  <Within entire survey>
  4. Click <Update>

  1. Click <Add logo>
  2. Click <Browse>
  3. Select the file <usalogomd>

  1. Page title type <Demographics>
  2. Page description <Answer All. Be as specific as possible>
  3. Click <Update>

8. Add Question 1 (Figure 12, 13)

  1. Click <Add Question>
  2. Click <Choose-one Answer (Vertical)>
  3. Type <Age>
  4. Customize Required
    • Click <customize required>
    • Change text for <All questions in the survey>
    • Click <use asterisk> for required fields
    • Click <update>
  1. Answer choices:

·        Type <Under 18, 18-40, 41-64, 65+>

  1. Check <Other (please specify)>

  1. Edit Question 1 - Select  <One Answer (Horizontal)>

  3. Click <Update> for both the questions

18. To Collect Responses (Figure 28-31)

            After designing your survey, you need to gather responses from the people whose opinions matter to you. Once you have collected responses, you cannot fully edit your survey.

  1. Click <My Survey> tab
  2. Click <Collect> icon, there are two collection methods

i.         Create link: e-mail message, web page, and email list.

ii.       Create Popup: invitation popup and survey popup

  1. Click <Create link for a web page>
  2. Copy the code and paste it on your website or e-mail address (Example shown on OLL website)
  3. Click <Done>
