College of Medicine Transcripts (Medical
School Students Only) - Contact the Office
of Student Records.
Transcripts are $6.00 per copy.
Official transcripts are issued by the
Registrar's office and bear the University seal and Registrar signature.
Official transcripts are not processed if the student has an outstanding
obligation, financial or otherwise. Holds can be checked through PAWS.
Students can obtain an unofficial transcript through PAWS.
Students may also update their mailing address through PAWS.
If for any reason, transcripts cannot be released you will be notified
by mail or email.
Requesting Official Transcripts
On-Line
The University of South Alabama has authorized
the National Student Clearinghouse to provide transcript ordering via
the Internet. To order an official transcript, logon to the Clearinghouse
secure site.
If you are a first-time visitor, you will
be directed to create a personal profile and user ID/Password, which
you will use to place future orders.
The site walks you through placing your
order, including delivery options and fees. You can order as many transcripts
as you like in a single session. A $2.25 processing fee will be charged
per recipient.
In order to ensure confidentiality and
to comply with federal regulations you must complete the Consent to
Release form. You will be instructed to print the form, sign and fax
it to the Clearinghouse before your order can be completed.

To request a copy of your USA transcript,
complete a Request for Transcript form
and mail or fax the request to:

Transcript requests should include:
- Student number or social security number
(voluntary)
- Name (name while at USA and any others)
- Date of birth
- Address where the transcript is to be sent
- Any special instructions
- Current mailing and e-mail addresses
- Your signature
- A check or money order for $6.00 per transcript
requested or credit card information (card type, number and
expiration date).

Walk In
Walk-in requests for transcripts can be
made at the service counter of the Registrar's Office located in Meisler
Hall, Suite 1100. Office hours are Monday-Friday 8:00 a.m. to 5:00 p.m.
General Information
Processing time generally takes two business days.
Faxed transcripts cost an additional $5.00 for domestic fax numbers
and $15.00 for international fax numbers.
Transcripts sent directly to a third party are "official".
If your transcript needs to be submitted as part of a packet or hand-delivered,
you need to request a self-managed package transcript. The self-managed
package is indicated with "issue to student for self managed package"
stamped on the transcript. It is placed in a sealed envelope marked
as "Official Document" on front. The flap of the envelope
has the Registrar's signature and is stamped "unofficial if opened
by student". The self-managed package will be determined as "official"
or "unofficial" by the recipient. The student is responsible
for determining the recipient's policy for self-managed packages.
To send a transcript other than by regular mail (e.g.,
Express Mail or Federal Express), authorization and additional fees
are required. If using USPS Express Mail, a pre-paid mailer must be
provided. If using Federal Express, a street address, not a P.O. Box,
must be provided along with credit card information. The University
of South Alabama cannot send transcripts using UPS or Airborne Express.
Picture identification is required to pick up a requested
transcript. A third party cannot pick up a transcript without written
permission from the student.
