New Students
Undergraduate and Graduate Applicants must submit both an Application Packet (PDF format) and a Financial Affidavit. (PDF Format).
NOTE: Health Services also requires an Immunization and TB Form (PDF Format) to be submitted.
Returning Students
Undergraduate and Graduate Applicants who have attended the University of South Alabama within the past year must submit only a Readmission Application (PDF Format).
Undergraduate and Graduate Applicants that have attended the University of South Alabama in the past, but over one year has passed since last registering for class, must submit the Financial Affidavit in addition to the Readmission Application.

Assistance and Delivery
For assistance on Undergraduate and Graduate admissions, or to get a hard copy of the application material via postal service, please e-mail a request to internationalservices@usouthal.edu.
Upon completion, the forms can be sent to University of South Alabama's Department of International Services via the following methods:
