The Department of Radiologic Sciences programs are set up in two parts. You must first complete the certificate program (or another college-based radiography program) in order to apply to the Baccalaureate degree program. If you are a registered radiographer (from a college-based radiography program) or you will graduate from your college-based program prior to the fall of the year you wish to enter the B.S. program, then click on the link below to go to the appropriate application. If you have NOT started a college-based radiography program, and wish to do so, use the "BACK" button on your browser to return to the forms page and click on the "Radiography Certificate Program Application."
