CP 200 Application

These forms are to be used for enrollment in CP 200, Clinical Observations. A new form must be completed each semester you wish to enroll in the course.

If you want to do the standard CP 200, complete the application form only, print and return to Dr. Stanfield.

If you want to take CP 200 with an outside clinician, you must first take the supervisor form to the outside clinician and get it signed. Then complete the application form, print and return to Dr. Stanfield.

CP 200 Evaluation

Print the evaluation form and take it to your CP 200 supervisor to complete and sign. The form must be returned to Dr. Stanfield by the last day of classes.

Mock Interview

Complete the mock interview form and bring it with you to your interview appointment. You may also bring a resume if you wish. The interviewer may keep the form for his/her records. A copy is generally returned to Dr. Stanfield with comments.

Committee evaluations are required to accompany applications to many health professions program. To start the committee evaluation process, you must complete the following form and return in to Dr. Stanfield. At such time, Dr. Stanfield will provide you with the evaluation forms you will need to take to those persons you choose to complete evaluations. Evaluators must include 3 science faculty (only 2 can be from the same department), 1 non-science faculty, and 1 from whomever you choose.
