Open Enrollment begins on November 1, 2009.  If you do not wish to make any changes to your health/dental insurance, no action is required. Employees should have received a newsletter that provided detailed information about benefit updates, premium changes and other items of interest. Insurance enrollment/contract change forms are available in the Human Resources Offices.  New premiums will be deducted from the paychecks in December since premiums are deducted one month in advance.

SouthFlex Forms are now available online and in the Human Resources Offices. You may complete, print, sign and fax the SouthFlex enrollment form to 460-7483.  Faxed copies of the SouthFlex enrollment form are accepted.  If you send by fax, it is not necessary to send the original.  Remember, you must enroll each year to participate in SouthFlex.  Even if you are currently enrolled, you must submit a new enrollment form to participate in 2010.

The deadline for submitting changes and enrollments is 4:30 p.m. on Monday, November 30.  After November 30, forms for open enrollment will not be accepted, and the web links for open enrollment will become inactive.
