OT Student Portfolio

USA students are required to maintain a record of assignments that demonstrates progress and improvement over the course of the curriculum.  This portfolio provides a permanent record of the student’s achievement and samples of their work.  These work samples are intended to be used as reference for fieldwork supervisors and when seeking employment.  Each student has a notebook that includes the following work samples:

            •           Assessments/Evaluation Summaries
            •           Intervention/Treatment Plans
            •           Progress Notes
            •           Discharge Summaries
            •           Group protocols/Home Programs
            •           Intervention/Treatment Activities
            •           Occupational Performance Analysis
            •           Presentations
            •           Papers
            •           Professional Correspondence
            •           Resources
            •           Fieldwork I & II Evaluations
            •           Faculty PDA and Self-Assessments
