In order to begin the process of applying for category 1 credit for your program or activity, fill out the following application completely and mail to the address indicated. Please contact us about fees before submitting an application.
Please note that as part of the certification requirements, NO publicity concerning this activity may be distributed without prior approval by the Department of CME. If any publicity, even a "save-the-date" card, has already been sent, call to see whether you are eligible to apply for credit.
We must receive your application no later than two months prior to the date of the proposed activity and eight to twelve (12) months if extensive planning is done by the CME office.
