Nurse Practitioners Needed

We are seeking certified Nurse Practitioners to provide ongoing outpatient coverage for in-home health assessments. Must have a minimum of 1-2 years of
experience.The NP will be responsible for the assessment of patients and will document and assess any situations that should be brought to the attention of the patient's Primary Care Physician, provide patient/family education, and make general recommendations intended to improve the members' knowledge of their chronic condition(s). Practitioner will not be responsible for prescribing medications.

The NP is provided training and the necessary equipment/medical supplies needed to perform the assessments. Assessment visit takes approximately
1.25-1.5 hours to complete. Collaborative physicians provided. Provider can work 8 or 10 hour days during business hours of 8a-6p. Appointments are
scheduled Monday through Saturday. Average 5-6 appointments scheduled for regular day. Must have minimum availability to work 40 hours per month (i.e.
four 10 hour shifts). (Full time and Part time hours available)

Requirements:
• Active unrestricted state license to practice
• Clean work history
• NP Certification from either ANCC or AANP.
• Geriatric or adult medicine specialty a plus but will also consider family practice and urgent care as long as experience with geriatric patient population.
• Minimum of 1-2 year experience
• Valid driver's license
• Must be able to effectively communicate with elderly and chronically ill patients and families
• Comfortable working independently in a home-visit setting
• BLS/CPR certification

This position requires travel to patients' homes so must be willing to travel within the agreed upon territory to conduct the home assessments via
