Completing your Graduation Application
Listed below are the instructions for completing your
graduation application. Please follow the instructions carefully. The
link to the application is found at the bottom of this page.
The form is provided in portable document format (pdf).
Adobe’s Acrobat Reader is required to view, complete, and print
the form. We recommend that you use Adobe Reader 8. If you do not already
have Acrobat Reader, click on the link below and download your free
copy of Acrobat Reader.

- Indicate your Jag number and level of degree.
- Indicate name to appear on your diploma. The names
you wish to appear on your diploma must be verified in the Registrar’s
Office.
- Select the term and enter the year for which you
are applying. This is the term that ALL degree requirements will be
met.
- Select your college. †
- Enter your program of study. †
- Enter your major. †
- Enter your minor(s). Note: Not all colleges require
a minor. †
- Enter your concentrations(s). Note: Not all programs
require a concentration. †
- Enter the degree(s) for which you are currently
applying (ex. BA, BS, MBA, MED). †
- Enter your catalog term. †
- Indicate participation in University Honors or Departmental
Honors Program(s). Undergraduate students only.
- Provide a Diploma Mailing Address.
- Read the Authorization information on the application
form; sign; date the form; and indicate a valid telephone number and
email address where you may be reached.
- Pay the $30.00 application fee ($15 reapplication
fee) to the Office of Student Accounting (Meisler Hall, Suite 1300)
and return the application to the Registrar’s Office (Meisler
Hall, Suite 1100).
Note: Mailed application with check/money order/credit
card information is sent directly to the Registrar’s Office,
USA, Meisler Hall, Suite 1100, Mobile, AL 36688-0002.
NOTE:
School of Continuing Education and Special Programs students must
have the application stamped by the School of Continuing Education
and Special Programs (AHE 214) before turning in your graduation application
to the Registrar’s Office.
