Completing your Graduation Application
Listed below are the instructions for completing your
graduation application. Please follow the instructions carefully. The
link to the application is found at the bottom of this page.
The form is provided in portable document format (pdf).
Adobe’s Acrobat Reader is required to view, complete, and print
the form. We recommend that you use Adobe Reader X. If you do not already
have Acrobat Reader, click on the link below and download your free
copy of Acrobat Reader.

- Enter your Jag number and level of degree. (Undergraduate,
Graduate, or First Professional/PhD)
- Enter name to appear on your diploma. The names
you wish to appear on your diploma must be on file and verified in
the Registrar’s Office.
- Mark the term and enter the year for which you
are applying. This is the term that ALL degree requirements will be
met.
- Enter your college. ‡
- Enter your major. ‡
- Enter your minor(s). Note: Not all programs
require a minor. ‡
- Enter your concentrations(s). Note: Not all
programs require a concentration. ‡
- Enter the degree(s) for which you are currently
applying. (BA, BS, MBA, MED, etc.) ‡
- Undergraduate students only. Indicate
participation in approved University Honors or
Departmental Honors Program(s).
- Read the Authorization information on the application
form; Print, sign and date the form;
and, provide a valid telephone number and USA email address where
you may be reached.
- Provide a Diploma Mailing Address.
The initial application fee is $50.00 and the reapplication
fee is $25.00. Submit the application to the Office of the Registrar.

TO SUBMIT YOUR APPLICATION (in
order of preference)
- Fax the completed
application to (251) 460-6123, or
- Email the completed
application, as an attachment, to graduation@southalabama.edu, or
- Mail application
with check/money order/credit card information to the University of
South Alabama, Registrar’s Office, 390 Alumni Circle, Suite
1100, Meisler Hall, Mobile, AL 36688-0002, or
- In person - submit
the application to our office.
Note: Interdisciplinary majors in
the School of Continuing Education and Special Programs and PhD students
in the College of Education must have the application endorsed by their
respective School or College before submitting the application to the
Registrar’s Office.
