Hooker-Kubic Endowment Award Program Applications
 
Application Information for Coming to the University of South Alabama
 
Award eligibility: The criteria to be used in awarding the Hooker-Kubik Scholarship are as follows:
 
For award amount go to the home page - subject to availability of funding each year.
 
Limit: recipients may only receive this award once in a given 2 year period.
 
The following application process is for funding for tuition/program expenses for one or two semesters (Fall, Spring and or Summer in any combination up to the limit of two semesters) for the following (in order of priority):
 
1. Applicants from Eastern Slovakia
OR
2. Applicants from Slovakia as nominated through written agreements by University sister universities in Slovakia - Award recipient priority will be given to students from Slovakia in the following order:
  a. Graduate degree seeking student including Masters, PhD, and Professional Degrees
  b. Undergraduate degree seeking student
  c. Non-degree seeking short term (one or two semesters)
 
Award Decision date: ALL AWARD ANNOUNCEMENTS WILL BE MADE IN WRITING in the Spring of each year or as announced.
 
Award recipients will only receive the funding when confirmation is received of their participation in the activity/program of choice. This funding will NOT cover 100% of tuition/expenses at USA applicants should have added sources of funding for travel to and their planned activity in the USA. Funding is also subject to the participant holding the appropriate and valid US or host country immigration status.
 
Application Process:
I. Download the appropriate application document. Document must be TYPED – no handwritten applications:

Complete Application A (MS Word form – FIRST SAVE FILE then complete)
II. Attach curriculum vitae with dates and listing of work experience and academic history.

III. Attach 3 letters of recommendation, letters must be in ENGLISH and must include contact information for the person writing the letter. At least one letter must be from a Dean or Department chairperson at your current or former university verifying your area of study and period of attendance.

IV. Attach a detailed proposal describing your planned activity under one of the eligibility criteria as outlined above. This proposal must be TYPED, with one inch (24.4mm) margins, and 12 point type. No longer than 5 pages.

V. Include in the proposal a brief budget explaining how the funds are to be used and the need for such funding.

VI. If you have been admitted to a USA degree program or invited to do a short term study at USA you must attach a copy of the admission notification or letter of invitation from the academic department. If you have not applied for admission or have not contacted a USA department for affiliation you will be required to do so prior to accepting the award.

COMPLETION OF THIS APPLICATION PROCESS DOES NOT CONSTITUTE ADMISSION TO USA OR TO YOUR HOST SITE. YOU MUST COMPLETE ANY REQUIRED DOCUMENTATION (INCLUDING IMMIGRATION/VISA APPLICATIONS, ETC.) RELEVANT TO YOUR PROPOSED ACTIVITY

A USA Council for International Education & Scholarship (USACIES) selection committee will review all applications received by the deadline. The committee reserves the right to contact the applicant for additional information, for interviews (phone, Skype, etc.) and to contact references as well as to verify all information submitted

Application Information for University of South Alabama Students
 
Award eligibility: The criteria to be used in awarding the Hooker-Kubik Scholarship are as follows:

Limit: recipients may only receive this award once in a given 2 year period.
 
The following application process is for funding for program expenses associated with a USA students studying abroad (study, research, internships, service learning) in the following order of DESTINATION priority:
1. USA study abroad student interested in study in Slovakia Flag Slovakia
2. USA study abroad students interested in study in Eastern Europe: Belarus Flag Belarus, Bulgaria Flag Bulgaria, Czech Republic Flag Czech Republic, Estonia Flag Estonia, Latvia Flag Latvia,
     Lithuania Flag Lithuania, Hungary Flag Hungary, Moldova Flag Moldova, Poland Flag Poland, Romania Flag Romania, Russia Flag Russia, Ukraine Flag Ukraine.
OPEN TO ALL MAJORS, ALL DISCIPLINES with a MINIMUM USA OVERALL GPA 2.5 and on programs
that are at least 6 weeks in length or longer (preference will be given to semester length programs).
 
Award Decision date: ALL AWARD ANNOUNCEMENTS WILL BE MADE IN WRITING in the Spring of each year or as announced.
 
Award recipients will only receive the funding when confirmation is received of their participation in the activity/program of choice. Funding is also subject to the participant holding the appropriate and valid host country immigration status.
 
Application Process:

II. Attach a copy of your USA transcript with current course work and GPA (an official copy may be requested for verification during the review process).

III. Attach 3 letters of recommendation, letters must be in ENGLISH and must include contact information for the person writing the letter. At least one letter must be from a Dean or Department chairperson, Academic Advisor or faculty member in your area of study.

IV. Attach a detailed proposal describing your planned activity under one of the eligibility criteria as outlined above. This proposal must be TYPED, with one inch (24.4mm) margins, and 12 point type. No longer than 2 pages.

V. Include in the proposal a brief budget explaining how the funds are to be used and the need for such funding.

VI. If you have been admitted to study abroad program or invited to do an internship, service learning or research abroad attach a copy of the any letter of invitation. Prior admission to your activity or program of choice is NOT required for consideration for this award however be aware that you should demonstrate strong knowledge of your intended program in your application.

Application Information for coming to the University of South Alabama
 
Award eligibility: The criteria to be used in awarding the Hooker-Kubik Award are as follows:

Limit: recipients may only receive this award once in a given 2 year period.
 
The following application process is for the funding of collaborative research activities including:
 
i. Research scholars FROM SLOVAKIA coming to USA for additional study, research activities, conferences/presentations, and related activities
OR
ii. USA undergraduate, graduate students and/or faculty for research and teaching activities IN SLOVAKIA.
 
Award Decision date: ALL AWARD ANNOUNCEMENTS WILL BE MADE IN WRITING in the Spring of each year or as announced.
 
Award recipients will only receive the funding when confirmation is received of their participation in the activity/program of choice. This award is not expected to cover all expenses for the planned activity, applicants should have added sources of funding for their planned activities. Funding is also subject to the participant holding the appropriate and valid US or host country immigration status.
 
Application Process:
I. Download the appropriate application document. Document must be TYPED – no handwritten applications:

II. Attach curriculum vitae with dates and listing of work experience and academic and publication history. Attach copies of any letters of invitation that you may have received from the hosting institution, faculty, etc.


III. Attach 3 letters of recommendation, letters must be in ENGLISH and must include contact information for the person writing the letter. At least one letter must be from a colleague familiar with the area of research, teaching, study, etc.


IV. Attach a detailed proposal describing your planned activity under one of the eligibility criteria as outlined above. This proposal must be TYPED, with one inch (24.4mm) margins, and 12 point type. No longer than 5 pages.

V. Include in the proposal a brief budget explaining how the funds are to be used and the need for such funding.
