As the sponsor of the USA Health & Dental Plans, USA Choice and USA Select, (“the Plans”), we are obligated to furnish you with certain documents related to the Plans and your benefits under the Plans. We would like to provide several of these documents to you electronically. The documents are:

Our intention, if you consent, is to provide you access to these documents electronically via the above web links. The documents are available here as PDF documents.
