Employee Portal for Form 1095-C

The University of South Alabama has partnered with Equifax to provide employees easy access to current year 1095-C forms and any prior year forms for tax preparation purposes. The Form 1095-C does not need to be filed with your tax return, but you should keep a copy of the form with your important tax documents.
