3.2 Group Life Insurance

Group Term Life and Accidental Death and Dismemberment insurance benefits are included in the Basic Group Life Insurance Plan. The amount of life insurance for which you are eligible is determined by your base pay, exclusive of overtime pay, shift differential pay, etc. Accidental Death and Dismemberment Insurance in an amount equal to the Basic Life insurance is also provided. These benefits are provided at no cost to you.

In addition to the Basic Group Term Life Insurance and the Accidental Death and Dismemberment coverages provided, your spouse and eligible dependents are each provided with $5,000 of term life insurance at no cost to you. Eligible dependents are your unmarried or married children from live birth to under 26.

An Additional Voluntary Group Term Life Insurance Plan is offered to all regular employees eligible for the Basic plan. This plan provides all eligible employees an opportunity to purchase additional amounts of term life insurance over and above the amount provided under the Basic Plan. Additional coverage must be medically underwritten if elected outside the guaranteed issue period. If approved, eligible employees may purchase, at their own expense, additional voluntary term life insurance in an amount equal to 1, 2, or 3 times their basic annual earnings not to exceed policy limits.
