How to Apply For A USA Identification (J) Number

The University welcomes all individuals interested in obtaining a USA identification (J) number to enroll today by completing the following forms:

• Individuals:
  
  • USA ID Number (J Number) Registration Packet for Individuals
    
    

• Domestic Companies:
  • USA ID Number (J Number) Registration Packet
    
    
• Foreign/International Individuals:
  • Form W-8BEN

• Foreign/International Entities:
  • Form W-8BEN-E

Please note: Vendors interested in submitting a bid to USA are not required to have a USA vendor identification number prior to the bid opening. A USA vendor identification number is only required for the awarded vendor before an initial Purchase Order can be executed.

Policy Change: Unsolicited paperwork, for obtaining a new USA vendor identification number, submitted to the Purchasing Office will no longer be accepted. Vendor’s may submit their paperwork to USA departments when requested.
