Benefits Eligibility

Basic Life, Additional Life, LTD & STD (STD only available for USA Health Employees) 

Employees must be in a regular position, working .75 FTE or greater, regardless of employment date to be eligible for the benefit. 

Closed grandfathered employees – A small group of employees will be grandfathered into these benefits that were previously Base plan eligible working a minimum of a .50 FTE up to .74 FTE. Any status change which results in an FTE reduction to less than .50 FTE or greater than a .74 FTE, the grandfathered status is lost. Future benefit eligibility will then require an FTE of .75 or greater. 

Health Insurance 

Employees must be in a regular position, working a .75 FTE or greater, regardless of employment date to be eligible for health insurance. 

Closed grandfathered employees: Any employee currently enrolled in health insurance with an FTE of .50 to .74 will remain eligible for the benefit. Any status change which results in an FTE reduction to less than .50 FTE or greater than a .74 FTE, the grandfathered status is lost. Future benefit eligibility will require an FTE of .75 or greater. 

Eligibility for an offer of health insurance for a temporary employee is based on total average hours worked per month (greater than 130 hours on average) during the lookback measurement period. Under this lookback measurement period, the employer measures an employee’s hours over a period of time, usually 12 months. After completing 12 months of employment within the lookback period, if the employee averages at least 30 hours per week (or 130 hours per month) over that measurement period, then he or she must be offered coverage for a “stability period” following the measurement period. The “stability period” begins January 1 and ends on December 31.
