Open Enrollment for the USA Health & Dental Plans and SouthFlex

Open Enrollment for the USA Health & Dental Plan (Base & Standard), USA Select Health & Dental Plan, and SouthFlex begins on Monday, November 1, 2021. If you do not wish to make any changes to your health/dental insurance, no action is required. Employees should have received the 2022 Open Enrollment/Rate Announcement and the Benefits & Wellness Open Enrollment Guide at their home address, which provides detailed information about benefit updates, and other items of interest. For new enrollments effective January 1, 2022, premiums will be deducted from bi-weekly paid employees in the month of December since premiums are paid one month in advance.

In order to participate in SouthFlex; you must enroll each year. Even if you are currently enrolled in SouthFlex, you must submit a new enrollment form to participate in the 2022 plan year.

The deadline for submitting changes and enrollments is 4:30 p.m. on Tuesday, November 30,2021. After this date, forms for open enrollment will not be accepted and the web links for open enrollment will become inactive. All changes and elections will become effective on January 1, 2022. ▼   USA Choice Plan & USA Select Plan

The USA Choice Plan & USA Select Plan enrollment/change form is available online and in the Human Resources offices. You may scan and e-mail the completed enrollment form to employeebenefitshr@southalabama.edu. Forms will not be accepted by fax. If you send your form by e-mail, it is not necessary to return the original form. 

The claims administration of SouthFlex will be managed by HealthEquity®. We also offer the HealthEquity® Visa® Reimbursement Account Debit Card. When you incur a qualified medical expense, you will be able to pay with the HealthEquity® Visa® Reimbursement Account Card or submit the expense through the HealthEquity online tool for reimbursement. Visit the SouthFlex page for more information. The SouthFlex Enrollment Form is also available online and in the

Human Resources offices.

Your USA Fringe Benefits Committee (FBC) recommended, and the USA Board of Trustees and Interim President Dr. John Smith approved, an employee premium increase of $4 per month for single coverage and $14 per month for family coverage for employees enrolled in the current Base and Standard plans for the 2022 plan year. The USA Select plan will have no increase in premiums for the 2022 plan year.

In addition to the modest employee premium increase, your USA Fringe Benefits Committee (FBC) recommended, and Interim President Dr. Smith approved, the following changes that have been adopted for the 2022 plan year:

• The Base and Standard Plans will blend into one plan effective January 1, 2022, called the USA Choice Plan. At this time, the premium rate structure will remain with a base and standard premium based on the employee’s date of employment.

• The out-of-pocket maximum for major medical expenses in the USA Choice Plan will increase from $1,850 to $2,250 for individuals and from $3,700 to $4,500 for families. 

USA Choice Plan - Base premiums: (employees hired before 1/1/2013):

Single coverage : $122 ( $172 without the non-tobacco wellness incentive)

Family Coverage : $408 ( $458 without the non-tobacco wellness incentive)

USA Choice Plan - Standard premiums: (employees hired on or after 1/1/2013):

Single coverage : $142 ( $192 without the non-tobacco wellness incentive)

Family Coverage : $472 ( $522 without the non-tobacco wellness incentive)

▼   New - Pharmacy Benefit

We are pleased to announce that effective January 1, 2022, Prime Therapeutics, Inc. (BCBS) will be the new pharmacy benefit manager for the USA Choice Plan (formerly USA Health & Dental Plan – Base & Standard plans). Prime Therapeutics, with direct ties to Blue Cross Blue Shield of Alabama, is a national leader in pharmacy management with a network of over 60,000 independent and chain pharmacies nationwide. Plan members can utilize any participating pharmacy to fill maintenance medications and other prescribed medications. Convenient home delivery services will be available from Meds Your Way™ through Amazon Pharmacy. Plan members will receive additional information at their home addresses regarding the change to Prime Therapeutics and in the USA Benefits & Wellness Open Enrollment Guide.

In December, you will receive a welcome letter that explains your benefits and offers additional information regarding all the prescription services available to you. You will also receive a new Blue Cross Blue Shield of Alabama member ID card which contains a “Rx” indicator. Beginning January 1, 2022, please be sure to present your new member ID card to your pharmacist when filling prescriptions.

▼   USA Select Plan

The University of South Alabama offers benefits-eligible employees the opportunity to participate in an alternate plan, the USA Select Plan, with claims administered by Blue Cross Blue Shield of Alabama. The USA Select Plan will offer value to employees and their dependents with a low-cost benefit plan that provides access to medical providers within USA Health but also provides the option of accessing the BCBS PPO network. The USA Select Plan Plan will have no increase in premiums for the 2022 plan year. 

Single coverage : $90 ( $140 without the non-tobacco wellness incentive)

Family Coverage : $300 ( $350 without the non-tobacco wellness incentive)

▼   Open Enrollment for Voluntary Group Term Life Insurance Sun Life

We are pleased to announce an opportunity for you to apply for voluntary group term life insurance at a modest cost from Sun Life. Voluntary life insurance is a simple, easy way to increase the financial protection of your loved ones.

During open enrollment only, Sun Life is excited to offer to current benefits-eligible employees, additional coverage in the amount of 1 times your basic amount, without proof of good health (medical underwriting). You may also apply for 2 or 3 times your basic life insurance with medical underwriting required. This benefit is in addition to the basic life insurance the University provides to you at no cost.

During open enrollment, you may also apply for additional dependent life insurance as a guaranteed issue. Coverage for your spouse is available in the amount of $25,000 at a monthly rate of $8.39. You may also elect $10,000 of additional child life insurance coverage for your eligible, dependent children at a monthly rate of $3.00, regardless of the number of children covered. Keep in mind that you must have a minimum of 1 times additional coverage on yourself to have the additional spouse and child coverage.

Voluntary group life insurance premiums will be deducted once per month from the first pay check of the month. Premiums are paid a month in advance. Bi-weekly paid employees will have premium deductions withheld from the December 10, 2021, check and monthly paid employees will have premium deductions withheld from the January 1, 2022 check. Coverage is effective January 1, 2022.

Open Enrollment begins on Monday, November 1, 2021, and ends on Tuesday, November 30, 2021. For more information, you may contact a Benefits Consultant at your Human Resources Office.
