The University’s Human Resources department (HR) has been designated to coordinate the requests of applicants and employees for reasonable workplace accommodations.

Request to claim an exemption from the COVID-19 vaccination requirement for medical reasons or sincerely held religious beliefs:

University of South Alabama employees may seek an exemption to the vaccination requirement for medical reasons or sincerely held religious beliefs using this form. The University may ask for other information as needed to determine if you are entitled to an exemption. Current employees, as applicable, must submit this form by no later than January 4, 2022.  This form can be delivered in person or via email. If the request is granted the employee is required to follow all University COVID-19 safety protocols as applicable.
