PASSAGE USA Application Process

A full application for PASSAGE USA will consist of the following parts. Each part is a clickable link to verify you have all parts of the application. Copies of the IEP, Transcript, attendance, and Eligibility can be received from the high school you attended.

Please print off the checklist, and verify you have completed all parts of the application. Any incomplete applications will be sent a follow up email with the parts missing of the application. Applicants will not receive an interview until all
parts of the application are complete.

• Signed student eligibility form
• Medical history
• Student & Parent/Guardian Memorandum of Understanding
• Reference form from Teacher
• Completed PASSAGE USA Personal Support Inventory
• Document from student describing how PASSAGE USA will help you
• Copy of most recent IEP and/or Transition IEP
• Copy of high school transcript or final report cards
• Attendance records
• Proof Intellectual Disability diagnosis (psychological evaluation and/or Notice & Eligibility Decision Regarding Special Education Services)
• Check made payable to the University of South Alabama for the application fee
